Refund policy

 

PHC & DMPO REFUND

Policy refund for PHC is the following: Applicant may ask for a full refund (minus the $25.00 application fee) within 3 days  from the application date by submitting a letter signed  by the principal applicant of the account stating the reason why applicant is cancelling the policy. Applicant should expect to receive his/her refund within 45 business days as of the date FHS received your request. We will notify the applicant once we have received and processed the refund.

 

Policy refund for DMPO is the following: Applicant may ask for a full refund (minus the $25.00 application fee) within 30 days  from the application date by submitting a letter signed  by the principal applicant of the account stating the reason why applicant is cancelling the policy. Applicant should expect to receive his/her refund within 45 business days as of the date FHS received your request. We will notify the applicant once we have received and processed the refund.

Florida Health Solution Corp. is a Florida licensed Prepaid Health Clinic & Discount Medical Plan Organization.

 

PHC (Prepaid Health Clinic): Florida Health Solution Corp. is a Florida licensed prepaid health clinic (PHC). Members will pay Nominal copayment when receiving PHC healthcare services.

 

DMPO (Discount Medical Plan Organization): This plan is not insurance. This plan provides discounts at certain health care providers for medical services. This plan does not make payments directly to the providers of medical services. The plan member is obligated to pay for all health care services, but will receive a discount from those health care providers who have contracted with the discount plan organization.
