Who is FHS?

Florida Health Solution Corp. (FHS) is a licensed Health Plan approved by the Florida Office of Insurance Regulation committed to your health since 2002. FHS is a Florida licensed Prepaid Health Clinic (PHC) & Discount Plan Organization (DPO).

As a PHC, FHS pays for services rendered at participating providers, members will pay a minimal copayment when receiving PHC healthcare services. Preventive panels at no cost to member, plan benefits are available based upon eligibility.

FHS is also a DPO. This plan is not insurance. This plan provides discounts at certain health care providers for medical services. This plan does not make payments directly to the providers of medical services. The plan member will pay for all health care services at a negotiated rate.

General Medicine

Copayments starting at just $5.00.

Specialists

Copayments start at just $20.00. These specialists require a doctors' referral and the appointment must be done through a FHS representative

Diagnostic Procedures

FHS offers our members basic laboratories and diagnostic procedures, medically necessary at no cost

Preventive Panels

We offer to our PHC members FREE benefits under Preventive Panels, at participating providers.

Prescribed Medication

We provide generic medications from $4.99 and brand names such as Walgreens and Walmart, among others

Dental

An annual basic cleaning at no cost, and dental procedures at fixed prices

Vision

An annual eye exam and basic frames at no charge

Laboratories

Under our Panels are included some laboratories tests at no cost, available at different moments of your membership year

Programmed Surgeries

Available to you for a moderate payment

Urgent Care

It includes a visit to a strategically located Urgent Care Center near you

Doctor at your Door

Is an Urgent Care Service that is available 24 hours a day, having a doctor come to the comfort of your home

Pregnancy Package

We offer pregnancy care and delivery packages with our highly experienced obstetricians

Bronze

From 0 - 12 months in our plans

With the BRONZE category you have access to a wide network of providers with very low co-payments since the first day. Establishing a Primary Care Physician (PCP) is very important, to know and monitor your general health.

Silver

From 13 - 24 months in our plans

After the first year (12 months) your Plan has obtained a SILVER category in our Loyalty Reward Program, and now you begin to enjoy the ADDITIONAL BENEFITS, which will increase qualitatively and quantitatively.

Gold

From 25 - 36 months in our plans

With two years active in the plan (24 months), now your membership has reached a GOLD category in our Loyalty Reward Program, and you will be entitled up to 4 of the procedures listed in the ADDITIONAL BENEFITS table.

Diamond

36 plus months in our plans

Congratulations! You have reached the DIAMOND category in our Loyalty Reward Program, because your plan has been active for 3 years (36 months) and in gratitude for your continued Membership FLORIDA HEALTH SOLUTION would like to offer you the best! Now you are entitled up to 5 of the procedures listed in the ADDITIONAL BENEFITS table, where you will find procedures such as MRI, CT SCAN, NUCLEAR MEDICINE, COLONOSCOPY, and others.Benefits without waiting time and completely without co-payment.
